COVID-19 VACCINES
TAzewell County COVID-19 Vaccine clinic information
TCHD is offering COVID-19 vaccines to people 5+ who live or work in Illinois at our TCHD Tremont location.
TCHD offers Pfizer, Moderna and Johnson & Johnson.
TCHD will offer vaccines by appointment only on Mondays from 8:30 am - 3:30 pm. Vaccines and boosters are available. Schedule your appointment or call 309-929-0294
Booster doses for Moderna, Pfizer, and Johnson & Johnson COVID vaccines are available at TCHD.
- Moderna - 2 dose vaccine is for 18+
- Pfizer - 2 dose vaccine for 5+
- Johnson & Johnson - 1 dose vaccine for 18+
The consent for 12+ is available. The consent for ages 5-11 is available. Please print and complete prior to arrival, if possible.
CHILDREN AGES 5-11 COVID VACCINE INFORMATION
TCHD will offer the COVID Vaccine to children ages 5-11.
Parent/adult must be with the child.
BOOSTER DOSES INFORMATION
Booster doses for Moderna, Pfizer, and Johnson & Johnson COVID vaccines are available at TCHD. Consent for booster dose is here.
All adults 16+ are eligible for a booster dose:
if you received a Johnson & Johnson vaccine at least 2 months ago..
if you received a second dose Moderna or Pfizer vaccine at least 5 months ago.
You may choose a different vaccine than you originally received.

- Peoria Civic Center Parking Lot at Fulton & Monroe (698 Fulton St, Peoria)
- Tuesday, Thursday, & Saturday
- 8:00 am - 4:00 pm
- Open to anyone regardless of symptoms, insurance, and immigration status will not be asked.
3rd Dose
Currently, individuals with moderately to severely compromised immune systems are recommended for a third dose of COVID-19 vaccine. You should receive the same brand of vaccine you received for your first and second doses, either Moderna or Pfizer-BioNTech.
